Most household poisonings are a matter of dose and time. Xylitol is a matter of physiology. In humans, xylitol has almost no effect on insulin — that is precisely why it is used as a sweetener. In dogs, the pancreas reads it as glucose and releases a large, fast slug of insulin in response. The insulin then clears real blood glucose that was never there in the first place, and blood sugar falls off a cliff.
The practical consequence is that a very small amount matters. Chocolate becomes dangerous in ounces; xylitol becomes dangerous in fractions of a gram per kilogram. A single piece of gum from a high-xylitol brand can put a ten-pound dog over the hypoglycemia threshold. And unlike chocolate, where signs build over hours, the drop here can begin before you have finished reading the packet.

Note what the second row implies. A dog can be treated for low blood sugar, look completely normal by the evening, and still be developing liver damage two days later. That is why veterinarians often recommend follow-up bloodwork after a large exposure even when the initial crisis resolves cleanly.

The peanut butter entry deserves emphasis. Peanut butter is one of the most common things owners hand a dog — inside a toy, wrapped around a pill, as a distraction during a nail trim. A handful of brands marketed as low-sugar or keto-friendly sweeten with xylitol, and the jar looks identical to the safe one on the next shelf. Reading the label once per brand, every time you buy, is the whole prevention strategy.
If the ingestion was recent and the dog is still asymptomatic, the first step is usually decontamination — inducing vomiting under supervision, which is safe only while blood sugar is still normal. From there, treatment is monitoring and support: serial blood glucose measurements over 12 to 24 hours, intravenous dextrose if the dog becomes hypoglycemic, and liver enzyme panels at 24, 48, and 72 hours for larger doses. Where liver injury is a real risk, hepatoprotectants such as S-adenosylmethionine and N-acetylcysteine are commonly used.
Activated charcoal, which helps for many toxins, does not bind xylitol well and is generally not the answer here. This matters because it is a common home-remedy suggestion online and it wastes the window in which something useful could be done.
Blood-sugar effects begin around 0.1 grams of xylitol per kilogram of body weight, and liver injury is associated with doses above 0.5 g/kg. For a 20 lb dog that first threshold is under one gram — which can be a single piece of gum from a high-content brand. There is no established toxic dose for cats, who seem far less affected, but there is no reason to test it.
Faster than almost any other common household toxin. Blood sugar can crash within 10 to 30 minutes of ingestion, though with some gum formulations the sugar-free base slows absorption and signs are delayed by up to 12 hours. Liver injury, when it happens, typically shows up over 12 to 72 hours. Because of the early window, this is a toxin where getting to a vet before symptoms appear genuinely changes the outcome.
The first signs come from hypoglycemia: vomiting, then weakness, stumbling or an unsteady gait, tremors, collapse, and seizures. Later liver failure presents differently — lethargy, jaundice (yellow gums or eyes), bruising, and abnormal bleeding. A dog can move from apparently fine to collapsed inside an hour, which is why the advice is to call before you see anything.
Not on your own initiative. If blood sugar has already started to fall, inducing vomiting can make things considerably worse, and hydrogen peroxide causes its own damage to the stomach lining. Call your veterinarian or a poison control line and follow what they tell you — they will decide based on how long ago the dose was and what your dog is doing right now.
The documented risk is much lower. Cats lack the same insulin-release response and reported cases are rare, most likely because cats do not taste sweetness and rarely eat sweetened products voluntarily. That is not an endorsement — if a cat has eaten a xylitol product, call your vet.
Usually just xylitol, but it may appear as 'birch sugar', 'wood sugar', or E967, and in the United States it can hide inside the umbrella term 'sugar alcohol' on some labels. Other sugar alcohols — erythritol, sorbitol, maltitol — do not cause the same insulin release and are not dangerous in the same way, so the specific word matters.
